After returning to Heaven, Metatron escapes from his restraints and impales the back of Naomi's head with her own drill.

When Cas arrives moments later, Metatron captures him and steals his grace , which is the last ingredient of the spell. While Dean tries to look for a way to exorcise the mysterious angel out of Sam's body, Castiel mentions how Crowley got into Samandriel's coding and theorizes that they could probably find a way to bypass the angel and talk directly to Sam. They go to Crowley to find out how to hack the angel's brain, but Crowley says that hacking an angel's brain is "more art than science" and offers to do it for them in exchange for going with them with the chains on.

The trio is eventually able to capture the angel and Crowley starts to jam spikes into his brain. After a while, Crowley is able to get the angel's name out of him: Gadreel. Cas identifies him as the angel who allowed Lucifer into the Garden to corrupt humanity. Unfortunately, Crowley is unable to bypass Gadreel, who mocks them about never finding Sam. This prompts Dean to get Cas to force Gadreel out of Sam's body.

However, Cas can't possess someone without their permission, so Crowley offers to do it in exchange for his freedom. Crowley possesses Sam's body and tells Sam's consciousness everything that's been happening. When Gadreel intervenes, they both fight back against him, ending with Sam ordering a defeated Gadreel to leave his body. After he leaves, Dean and Cas remove the spikes from Sam's head and the trio leave Crowley behind in the warehouse when Abaddon arrives. At a pier, Cas heals Sam of the injuries to his head.

When Efram and Jonah are unable to get Castiel to tell them where Sam and Dean are, they decide to hack his brain. Without Gabriel, the angels will burn out in time, Heaven will crumble and all of the souls in Heaven will fall back to Earth.

In Byzantium , Naomi appears to greet Castiel and Duma inside of Jack's Heaven and reveals that The Shadow has come in search of the Nephilim , seeing the him as its property and wanting to bring him to The Empty.

In Absence , when Castiel travels to the Heavenly Portal to find Mary Winchester 's soul , he tries to call Naomi over angel radio but gets no answer. When Castiel continues to call out, Duma appears and tells him that Naomi's location is none of his business.

However, Duma allows Castiel into Heaven briefly to see that Mary is at peace. In Jack in the Box , its revealed that following the Invasion of Heaven , Duma overthrew Naomi and became the Ruler of Heaven herself, using the fact that Heaven was invaded under Naomi's watch and the angels nearly died under her watch. Naomi is now in a very small cell which Duma is surprised that Castiel is not pleased with as he and Naomi weren't fond of each other.

Duma established a reign of terror in which humans were beat into submission, Heaven was stripped of its mercy and Jack was manipulated to solidify her own power unlike Naomi who was focused on keeping Heaven running for as long as possible.

Duma's reign of terror was ended when she was killed by Castiel for threatening the souls of John and Mary Winchester. Naomi appeared civil and normally friendly, and was willing to be diplomatic when it suited her.

She also made an effort to try and create alliances. However, she was also incredibly ruthless, and was more than willing to murder, torture, kidnap and brainwash others. She valued order and discipline, and worked hard to preserve these qualities. She held angels somewhat highly, and worked for their benefits, but that would not stop her killing them if she felt there was the need. When it came to humans , Naomi was less caring and was willing to savagely murder and torture dozens of them simply to further her goals.

However, she was also willing to be civil with them, especially if it benefited her. Like many angels , she hated demons and looked down upon them, although Crowley seemed to imply they had a relationship some thousand years ago in Mesopotamia. Despite this, Naomi still would not strike a deal with the demon, attesting to her strict and businesslike nature.

She longed for the gates of Hell to be closed, to the point where she helped the Winchesters to finish the second trial and insisted they could trust her because of that. However, after her change of heart, she revealed that closing the gates would kill Sam, even though she knew it would cause Dean to stop the trials, indicating that with her change of heart she became more compassionate and she developed the desire to prevent others from being sacrificed when there was another way.

An angel named Nathaniel approaches Naomi with the news that one of their freelance operatives has spotted Castiel in Houston, Texas. To Naomi's shock, Nathaniel reveals that Metatron has been spotted with him. Naomi and two angels travel to Houston and capture Metatron and face off with Castiel. Naomi orders an angel to kill Castiel, but after the bartender shoots the angel and Metatron tells Castiel to stand down, she just leaves with her henchmen and Metatron.

In her office in Heaven , Naomi straps Metatron to a chair. Despite having never met in person, Metatron recognizes her as the angel that was supposed to "debrief" him after God left Heaven. Naomi tells him that the archangels wanted to know the secrets in his mind and she now wants to know why he came out of hiding and what his plans are. Naomi drills into Metatron's mind and is shocked by what she finds, asking him why he is going with the plan that he has.

Metatron tells her that he was elevated to God's Scribe and it was incredible, but he was forced from his home and now wants payback. Horrified, Naomi leaves, leaving Metatron alone. Naomi appears on Earth near where Castiel and Dean are after having just completed the second trial to close the Gates of Heaven.

Castiel angrily confronts her with his angel blade , but Naomi insists that she is just there to talk, not fight and Dean gets Castiel to hear her out. Naomi reveals what she has learned from Metatron : as revenge for his expulsion from Heaven , he is using Castiel to expel all angels permanently, not close the gates of Heaven.

Castiel doesn't believe her, but Naomi insists that its true. She expresses regret for all of her actions, crying and tells Castiel and Dean that angels were meant to protect humanity and somewhere along the line they lost sight of that.

Naomi, reminding Dean that she told him he could trust her, tells him that while she wants him to close the Gates of Hell , she has learned that doing so will kill Sam , that God intended for the person closing the gates to have to make the ultimate sacrifice.

Naomi tells Castiel that if he wishes to return to Heaven she is now willing to hear him out and disappears. Castiel doesn't believe her claims and Kevin Tran is unable to back them up with the angel tablet , but Dean does and has Castiel take him to Sam. Castiel travels to Naomi's office in Heaven only to find that Metatron has impaled her in the head with her own drill.

Metatron confirms that Naomi told him the truth about his intentions. After Ezekiel tells him that a faction of angels is gathering power and vessels, Dean asks him if Naomi is their leader, but Ezekiel tells him that he hasn't heard her name mentioned.

Naomi reveals herself to be alive and one of the few remaining angels in Heaven. She tells a questioning Castiel that her drill was incapable of killing her, but the brain damage it caused gave her amnesia. She had a few trustworthy angels spread the news that she was dead while she has been recovering for the past five years and reassembling her memories.

Despite still having a few holes in her memory, she claims that she has to be in Heaven.
